Challenge

In the United States, approximately one in every three patients returns to the hospital within 30 days of discharge. The first 48 hours after any medical intervention are critical. This is when patients need the most help. Our client, Dimer Health, specializes in transitional care and aims to provide patients with proactive, personalized post-discharge care to ensure their smooth recovery. The company wanted to create a transitional care app with AI-powered recommendations that would reduce unnecessary hospital visits and make it easy for patients to coordinate care. To build this healthcare companion app, Dimer Health needed top expertise in HIPAA compliance, EMR integration, and AI technologies. That's when they called us, ITRex.

Our task was to:

ITRex designed and developed a user-friendly healthcare solution that provides the functionality needed to support the client’s clinical operations and satisfy patients’ needs. We assisted the client with the HIPAA compliance process. We helped them obtain the HIPAA Seal of Good Faith, a testament to their commitment to health data protection. ITRex also helped Dimer Health design and implement HIPAA-compliant IT infrastructure and application layer. The resulting solution integrates with EMRs and call center software. It includes the following functionality:
